Healthy Tri-County is a regional health coalition who seeks to improve the health and wellbeing of Berkeley, Charleston, and Dorchester Counties under the leadership of MUSC, Roper St. Francis Healthcare, Trident Health and Trident United Way and with partnership of over 70 community organizations. The Tri-County Community Health Needs Assessment Survey is an anonymous online survey that collects data directly from these communities to understand their greatest health-related needs.
This data is collected every three years and used to guide Healthy Tri-County and it's partners in decisions made about health programming in our community. For our hospital systems (Roper St. Francis Healthcare, MUSC, and Trident Health) this data is critical to guiding their funding for community health. For other Healthy Tri-County partners, this data is utilized to create an action plan called the Community Health Improvement Plan, which details how Healthy Tri-County aims to address the identified needs.
